By requirement of most insurance plans, payment of your copay, co-insurance amount, or deductible is expected at the time of service.  Statements are mailed monthly and prompt payment is appreciated.

Upon each visit to our office, you will be asked to inform our staff of any changes to your address, telephone number, or insurance coverage.  Please bring your insurance card each time you are seen.  You may also call or e-mail us with any changes. 

If you are preparing to come for your initial appointment, or have recently changed insurances, please review your insurance policy requirements to determine whether you need a referral from your primary care physician.  It is also wise to call before you arrive, to confirm that all needed referrals have been received.
